IMPORTANT HEALTH INFORMATION
According to Pennsylvania school regulations, students are required to have specific health records and examinations on file at school. Each grade has different requirements as per the Department of Health.
The following are required for each grade level:
Preschool- Physical Exam, Dental Exam, Required Age-Appropriate Immunizations
Kindergarten- Physical Exam, Dental Exam, Required Age-Appropriate Immunizations
1st grade- None required
2nd grade- None required
3rd grade- Dental Exam
4th grade- None required
5th grade- None required
6th grade- Physical Exam (due by the end of 6th grade year)
7th grade- Dental Exam (due by the end of 7th-grade year)
*** Immunizations Tdap & Menactra (Meningitis) - MUST be given prior to the first day of school
8th grade- None required
9th grade- None required
10th grade- None required
11th grade- Physical Exam (due by the end of 11th-grade year)
12th grade- *** Immunization: Menactra (Meningitis) - MUST be given prior to the first day of school
*All new students regardless of grade need to provide proof of immunization
It is important to keep in mind these requirements for the grade level that your student will be entering the following school year. Physicians are aware of what immunizations are required for your child’s age. If you have not provided the mandatory documentation for your student, please do so.
Completed forms can be emailed, faxed, or dropped off. Physical and dental forms are available on the district website.Elementary
